Uninsured patients sue Chicago nonprofit hospital

A lawsuit filed Thursday claims a nonprofit hospital in northwest Chicago failed to provide charity care to two low-income, uninsured patients.

The case reopens a longstanding controversy in Illinois over whether hospitals are doing enough charitable work to qualify for lucrative tax exemptions.

The lawsuit claims Swedish Covenant Hospital repeatedly lost one patient's financial assistance application and threatened to send her bill to a collection agency.

It states the hospital incorrectly told another patient she was ineligible for assistance and demanded cash from her.

Swedish Covenant spokeswoman Leigh Ginther says every patient who is identified as uninsured is given an application for charity care and a personal explanation of the process. The hospital reported $6.2 million in charity care expenses last year, nearly 3 percent of its net revenue.

Senate votes to tighten sanctions on Iran

(AP) ? Ignoring White House opposition, the Senate voted overwhelmingly Friday to impose tough new sanctions on Iran's domestic industries as it targets the Islamic Republic's economy to thwart its nuclear ambitions.

The 94-0 vote for a package of penalties reflected fears about the threat of Iran's nuclear program and the strong desire to protect the United States' closest Mideast ally Israel. Shortly before the vote, the powerful American Israel Public Affairs Committee urged senators to back the measure.

Sens. Bob Menendez, D-N.J., and Mark Kirk, R-Ill., sponsored the amendment to a sweeping defense bill that the Senate hopes to finish next week. The punitive measures build on sanctions on Tehran's oil and financial industries that Menendez and Kirk shepherded through Congress in the past year.

"The most recent sanctions are working toward crippling Iran's economy but Iran hasn't quit trying," Menendez said on the Senate floor.

Said Sen. John McCain, R-Ariz.: "The screws need to be tightened."

The sanctions would designate Iran's energy, port, shipping and ship-building sectors as entities of proliferation and sanction transactions with these areas. The legislation also would penalize individuals selling or supplying commodities such as graphite, aluminum and steel to Iran, all products that are crucial to Tehran's ship-building and nuclear operations.

In a memo from the National Security Council just hours before the vote, the administration argued that the new sanctions were unnecessary, duplicative and "threaten to confuse and undermine" provisions in current law.

Specifically, the administration complained about the omission of waivers that would give the president more flexibility, ambiguities that would make implementation difficult and requirements for congressional reports on thousands of small and mid-size vessels that dock at Iranian seaports that were too burdensome.

The Associated Press obtained a copy of the memo.

Senate Armed Services Committee Chairman Carl Levin, D-Mich., acknowledged the administration's reservations and said he hoped lawmakers could address those concerns in crafting the final version of the bill.

The legislation also would designate the Islamic Republic of Iran Broadcasting and its president as human rights abusers for broadcasting forced televised confessions and show trials.

The United States and European Union have imposed tough sanctions on Iran that have weakened its economy. But Tehran has found ways to bypass the penalties, such as Turkey's use of gold to pay for Iranian natural gas imports.

The Menendez-Kirk measure would allow the president to impose sanctions in cases of the sale or transfer of precious metals, targeting efforts by Iran to circumvent the penalties.

Iran maintains that its nuclear program is for peaceful purposes only.

The president has 90 days from the legislation's enactment to act. The bill does include the authority to waive the sanctions based on national security.

The $631 billion defense policy bill for next year authorizes money for weapons, ships, aircraft and a 1.7 percent pay raise for military personnel. The total is $4 billion less than the House-passed bill, and House-Senate negotiators will have to work out the difference in the closing days of this year.

Late Thursday, the Senate revived a divisive debate over civil liberties and the president's powers as commander in chief, voting that Americans citizens suspected of terrorism and seized on U.S. soil may not be held indefinitely.

A coalition of liberal Democrats and libertarian Republicans backed an amendment that said the government cannot detain a U.S. citizen or legal resident indefinitely without charge or trial even with the authorization to use military force or a declaration of war.

The Senate voted 67-29 in support of the measure sponsored by Sen. Dianne Feinstein, D-Calif., chairwoman of the Senate Intelligence Committee, and Sens. Rand Paul, R-Ky., and Mike Lee, R-Utah. The strong bipartisan approval sets up a fight with the House, which rejected efforts to bar indefinite detention when it passed its bill in May.

Ignoring a White House veto threat, the Senate also voted Thursday to add to its restrictions on President Barack Obama's authority in dealing with terror suspects.

Lawmakers approved an amendment that would prevent the transfer of detainees held at Guantanamo Bay, Cuba, to prisons in the United States. The vote was 54-41, with several Democrats vulnerable in the 2014 elections voting with Republicans.

Sen. Kelly Ayotte, R-N.H., argued that the 166 terror suspects, including Khalid Sheikh Mohammed, the self-styled mastermind of the Sept. 11, 2001, attacks, should remain at the U.S. naval facility and not be transferred to any facility on American soil.

Responding to Ayotte, Feinstein said the United States not only can but has handled terrorist suspects, with 180 now languishing in super maximum prisons. Feinstein complained that the measure would erase the president's flexibility.

In fact, the administration, in threatening to veto the bill, strongly objected to a provision restricting the president's authority to transfer terror suspects from Guantanamo to foreign countries. The provision is in current law.

Last year, Congress' approach to handling terror suspects divided Republicans and Democrats, pitted the White House against lawmakers and drew fierce opposition from civil liberties groups.

Current law denies suspected terrorists, including U.S. citizens seized within the nation's borders, the right to trial and subjects them to the possibility they would be held indefinitely. It reaffirms the post-Sept. 11 authorization for the use of military force that allows indefinite detention of enemy combatants.

That generated a conservative backlash as well as outrage among civil liberties groups.

In arguing for her amendment, Feinstein recalled the dark days of World War II when the United States forcibly removed thousands of Japanese-Americans and placed them in permanent internment camps amid unfounded fears that they were spies and national security threats.

House Republicans indicated they would stand firm.

Driven by the strong demand for high-definition video, digital health services, the Internet of Things, and virtual reality, broadband, ubiquitous and convergent information access has become the most important engine to drive the development of the modern information society. With increasing numbers of information-based interactions among humans, machines and objects, especially as new services, new terminals and new needs emerge, the networks are required to provide flexible, energy-efficient, safe and broadband access services anywhere at any time, and therefore wideband and ubiquitous information access has become the great demand of the modern information society. However, it is difficult to meet the growing demand with the existing technologies, so new solutions must be explored as a matter of urgency. The intelligent radio-over-fiber (I-ROF) system, which combines the advantages of flexible wireless access and fiber-optic broadband transmission, uses the methods of microwave photonics to realize the generation of multi-band, multi-standard microwave signals in the optical domain, along with broadband processing, large dynamic transmission, fast access and reconfigurable networking, and can thus provide an effective way to achieve broadband and ubiquitous access.

With the support of the National Program for Key Basic Research Project of China (973 Program, Grant No. 2012CB315705) and the National High-Tech R&D Program of China (863 Program, Grant No. 2011AA010306), a research group led by Professor Ji YueFeng, who is with Beijing University of Posts and Telecommunications and who is also the Chief Scientist of the National 973 Program, have focused on I-ROF systems and have studied the fundamental principles, network architecture and enabling technologies of I-ROF systems from the viewpoints of the required modules, system applications and networking. Also, a broadband access and ubiquitous sensing oriented, large dynamic, reconfigurable, and distributed I-ROF system experimental platform has been built to realize broadband wireless access applications. The group's work, entitled "Large dynamic, reconfigurable, distributed intelligent radio-over-fiber (I-ROF) system", was published in SCIENCE CHINA Information Sciences, 2012, vol. 42 (10).

The research group focused on the large demand for broadband access and for ubiquitous sensing for the Internet of Things and other applications, and proposed a large dynamic and reconfigurable distributed I-ROF system, which can meet this great demand. Also, the fundamental principles, network architecture and enabling technologies for these I-ROF systems in terms of modules, system applications and networking have also been studied. From the viewpoint of the modules, broadband and multi-standard microwave/millimeter-wave band vector signal generation, instantaneous photonic microwave frequency measurements, broadband and tunable microwave photonic filters based on photonic crystals, and broadband, high efficiency electromagnetic band gap structured antennas were investigated. For system applications, a large dynamic ROF system, a cognitive, collaborative and power efficient ROF system, and an optical and wireless resources joint management I-ROF system were covered. For networking, the network architecture of the I-ROF and the media access control (MAC) protocol for the distributed ROF network were studied. Based on the results of these studies, a broadband access and ubiquitous sensing oriented, large dynamic, reconfigurable, and distributed I-ROF system experimental platform was built to realize broadband processing of multi-band, multi-standard microwave signals in the optical domain, large-scale dynamic transmission and reconfigurable networking.

This new generation of I-ROF systems is typically representative of microwave photonics, which is broadband and oriented toward ubiquitous information access, and has the advantages of broadband operation over the full frequency band, a reconfigurable architecture and easy scalability. It also allows low operational energy consumption. I-ROF is therefore an appropriate direction for future development and has broad application prospects.

Rogers Communications Teams with SamKnows to Test Broadband ...

Rogers Communications has signed an official agreement with SamKnows, the ?leading authority on broadband service availability and independent broadband performance testing,? to measure the performance of its broadband Internet speeds.

The testing will be based on customer data collected in the home and will back up roughly $10 billion in spending from Rogers with respect to infrastructure.

?With Internet usage exploding, speed is more important than ever to the customer experience,? said John Boynton, EVP and Chief Marketing Officer, Rogers Communications. ?We are committing to long-term in-home testing as an additional quality measure to ensure customers are getting world-leading Internet experiences.?? We encourage other ISPs to do the same.?

Rogers will be looking to measure customer Internet speeds on a consistent basis to get the most accurate numbers it can. This will involve the use of a Whitebox to plug into existing Internet connections. From the Whitebox, Rogers and SamKnows will be able to measure the upload and download performances of the broadband connections and can monitor things like latency as well.

The Whitebox is a custom hardware device that SamKnows says should be installed to the existing router. It will run a series of tests and apparently doesn?t collect any other data beyond broadband performance information.

Egypt's political foes dig in their heels

CAIRO (AP) ? Supporters and opponents of Egypt's president on Sunday grew more entrenched in their potentially destabilizing battle over the Islamist leader's move to assume near absolute powers, with neither side appearing willing to back down as the stock market plunged amid the fresh turmoil.

The standoff poses one of the hardest tests for the nation's liberal and secular opposition since Hosni Mubarak's ouster nearly two years ago. Failure to sustain protests and eventually force Mohammed Morsi to loosen control could consign it to long-term irrelevance.

Clashes between the two sides spilled onto the streets for a third day since the president issued edicts that make him immune to oversight of any kind, including that of the courts.

A teenager was killed and at least 40 people were wounded when a group of anti-Morsi protesters tried to storm the local offices of the political arm of the president's Muslim Brotherhood in the Nile Delta city of Damanhoor, according to security officials.

It was the first reported death from the street battles that erupted across much of the nation on Friday, the day after Morsi's decrees were announced. The officials, who spoke on condition of anonymity because they were not authorized to speak to the media, identified the boy as 15-year-old Islam Hamdi Abdel-Maqsood.

The tensions also dealt a fresh blow to the economy, which has suffered due to the problems plaguing the Arab world's most populous nation since Mubarak's ouster.

Egypt's benchmark EGX30 stock index dropped 9.59 percentage points Sunday in the first trading session since Morsi issued his decrees. The losses were among the biggest since the turbulent days and weeks immediately after Mubarak's ouster in a popular uprising last year. The loss in the value of shares was estimated at close to $5 billion.

The judiciary, the main target of the edicts, has pushed back, calling the decrees a power grab and an "assault" on the branch's independence. Judges and prosecutors stayed away from many courts in Cairo and other cities on Sunday.

But the nation's highest judicial body, the Supreme Judiciary Council, watered down its opposition to the decrees on Sunday. It told judges and prosecutors to return to work and announced that its members would meet with Morsi on Monday to try to persuade him to restrict immunity to major state decisions like declaring war or martial law or breaking diplomatic relations with foreign nations.

Morsi supporters insist that the measures were necessary to prevent the courts, which already dissolved the elected lower house of parliament, from further holding up moves to stability by disbanding the assembly writing the new constitution, as judges were considering doing. Both the parliament and the constitutional assembly are dominated by Islamists. Morsi accuses Mubarak loyalists in the judiciary of seeking to thwart the revolution's goals and barred the judiciary from disbanding the constitutional assembly or parliament's upper house.

Opposition activists, however, have been adamant since the crisis first erupted that they would not enter a dialogue with Morsi's regime before the decrees are rescinded.

Protesters also clashed with police at Cairo's Tahrir Square, the birthplace of the mass protests that toppled Mubarak, and in the side streets and avenues leading off the plaza. The Interior Ministry, which is in charge of the police, said 267 protesters have been arrested and 164 policemen injured since the unrest began a week ago, initially to mark the anniversary of street protests a year ago against the nation's then-military rulers. Forty-two protesters were killed in those demonstrations.

The ministry did not say how many protesters were injured in the latest clashes in Tahrir, but security officials put the figure at 260.

Hundreds of protesters staging a sit-in in Tahrir have vowed not to leave before Morsi rescinds his decrees. The two sides also have called for massive rival protests Tuesday in Cairo, signaling a protracted struggle.

Morsi's office issued an English-language statement late Sunday defending his decrees and repeating the argument he used when addressing supporters Friday outside his Cairo palace ? that they were designed to bolster the country's transition to democratic rule and dismantle Mubarak's old regime.

"The presidency reiterates the temporary nature of the said measures, which are not meant to concentrate powers," it said. The statement also pledged Morsi's commitment to engaging all political forces on the drafting of a new constitution. Secular and Christian members withdrew from the panel drafting the document, alleging that the Islamists who dominate the body have hijacked the process to produce a charter with an Islamist slant.

Nader Omran, of the Brotherhood's Freedom and Justice party, said any changes to the decrees were "out of the question" but Morsi might "make pledges, vows or addendums explaining his position to clarify the decrees."

He said Morsi would hand over his legislative powers to parliament's upper chamber once a new constitution is adopted in a nationwide referendum and ahead of parliamentary elections. The presidency did not confirm the claim, but doing so would leave the Egyptian leader with the state's executive and legislative powers until around April.

The move also has caused some internal discord among the Morsi camp, with one aide, Samer Marqous, already resigning to protest the "undemocratic" decree.

Another Morsi adviser Ayman al-Sayyad wrote on his Twitter account that the president met Sunday for the second time in as many days with his 17-member advisory council and three of his assistants.

"I think it (the meeting) produced a genuine realization of the gravity of the situation ... We were candid today in our meeting with the president and we now expect practical steps on the ground." He did not elaborate.

Leading Egyptian democracy advocate Mohammed ElBaradei, a Nobel Peace Laureate and a former director of the U.S. nuclear agency, warned Saturday of increasing turmoil that could potentially lead to the military stepping in unless Morsi rescinds his new powers.

Egypt's liberal and secular groups ? long divided, weakened and uncertain as Islamist parties rose to power in the months after the revolution ? are seeking to rally in response to the decrees as the stakes are high. The fractured opposition came together as the engine of the 18-day uprising that forced Mubarak to step down after 29 years in power, bringing millions out on the streets, but they have failed to recapture that energy after months of setbacks.

Morsi, meanwhile, already has backed down twice in high-profile battles with the judiciary in the five months he's been in office. The U.S.-trained engineer failed to challenge the court ruling dissolving parliament's powerful lower chamber and had to rescind his decision to fire the country's attorney-general, Mubarak-era appointee Abdel-Maguid Mahmoud, in October. He fired him on Thursday as part of his edicts.

According to the Centers for Disease Control's National Survey of Family Growth, the divorce rate for first marriages was at its highest -- 40 percent -- in 1980, but has since declined to less than 30 percent. The divorce rate for college-educated couples who marry at age 25 or older is even lower, at 20 percent.

This is a dramatically different picture than what the media would have us believe. Rather than viewing marriage as a 50-50 shot in the dark, it can be viewed as having a 70-80 percent likelihood of succeeding.

So, how do marriages today stand the test of time? I've gathered some useful insights from marriage therapists, life coaches and married couples that have been together for over 25 years. Here are 10 ways they say will help affair-proof your marriage:

  1. Agree that marriage is a commitment. At the end of the day, marriage is about commitment. Whether it's viewed as a sacred bond or a civil contract, a marriage that is embraced by both spouses as a binding agreement -- not to be broken by sickness, financial hardship, or the temptation of an attractive co-worker -- is more likely to survive. These couples are committed to working through conflict together, without fear that the other person will jump ship when the going gets rough.
  2. Meet your partner's emotional needs. Simply put, a husband and wife must learn how to make each other happy -- and how to stop making each other unhappy. Dr. Willard F. Harley, Jr, marriage therapist and author of the book, His Needs, Her Needs: Building An Affair-proof Marriage, starts his therapy sessions by asking the couple: "What could your spouse do for you that would make you the happiest?" He has identified the 10 most common "emotional needs" that couples want met: admiration, affection, conversation, domestic support, family commitment, financial support, honesty and openness, physical attractiveness, recreational companionship and sexual fulfillment. Not surprisingly, the woman's top needs are often the opposite of the man's top needs. For example, conversation and family commitment top the woman's list of needs, while sexual fulfillment and admiration are at the top of the man's list. Understanding what your spouse's needs are -- and how to fulfill those needs -- will lead to a happier marriage.
  3. Be willing to negotiate. Being able to understand and respect your spouse's perspective, especially during conflict, is critical in a relationship. For example, one husband I spoke with needed more time alone together with his wife. He wanted her companionship. She, on the other hand, felt that their time as a couple should be spent with the children, as a family. Resolving this issue involves understanding and respecting the other person's feelings and being willing to compromise to satisfy both partners' needs.
  4. Learn how to fight. Resolving conflict in a way that meets each other's emotional needs and accommodates each other's feelings is critical in making a relationship last. Whether your style is to yell and shout, or to write letters to each other expressing your feelings, doesn't matter. What matters is finding a communication style that works for both of you. Bottling up anger and hiding issues will only lead to bigger problems down the road.
  5. Schedule time together. When I had my first child, a friend of mine with grown children gave me this piece of advice: she told me to schedule a weekly date night, and to hold fast to it no matter the circumstances. Four kids and 14 years later, my husband and I still have our weekly date night. Sometimes it's at night, sometimes during the day when the kids are in school. It doesn't matter. What does matter is scheduling time when you and your spouse can give each other undivided attention -- away from the kids and other couples. Dr. Harley seconds this advice and suggests that couples give each other a minimum of 15 hours alone together each week. In that time, says Dr. Harley, you should focus on the four emotional needs of affection, sexual fulfillment, intimate conversation, and recreational companionship.
  6. Make time for romance. The definition of "romance" for men and women is very different. Romance for most men means sex and recreation; for most women it's affection and conversation. Try combining all four of them for the greatest satisfaction. Women often resent having sex without affection and conversation first, and men resent being conversant and affectionate with no hope for sex or recreation.
  7. Keep expectations in check. Marriage isn't a fairy tale. If you go into it thinking that it's going to be easy sailing, you're setting yourself up for disappointment. Every marriage experiences conflict. The ones that succeed are the ones that develop healthy coping strategies to deal with problems. Viewing marriage as a working partnership, not just a romantic relationship, helps put things in perspective. Not that romance can't be part of the working partnership -- it can and should be. Just keep expectations in check and don't compare your marriage to what you see in the movies.
  8. Put marriage at the top of the list. With everything that couples have on their plates today -- kids, careers, ailing parents, financial stresses -- it's hard to put marriage first. It may be politically incorrect to say this, but marriage must come first -- even before your kids. A couple that's been married for 50 years told me that they always spent the first 20 minutes when the husband came home from work alone together, talking and having a cocktail. The kids had to go in the other room. It's okay to show your kids that your marriage comes first.
  9. Develop your own interests. Relationships that last have something interesting in common -- each partner has developed interests of their own, independent of their spouse. It might be a job, volunteer work, a book club, or a regular girls' night out. According to Dr. Noelle Nelson, relationship expert and author of Your Man Is Wonderful, having a part of your life that is your own can boost your self-confidence, which in turn can help your marriage. Along these lines, Dr. Nelson advises couples to take care of their physical health, not only to please their spouse, but to boost their self-esteem. Women worry too much about aging, she says. They get confused, believing that affairs happen because they get older and become less physically attractive to their spouses. Remember, confidence is attractive.
  10. Show your appreciation. Having an "attitude of gratitude" can go a long way in keeping a marriage intact. Couples often let appreciation slide because they assume, after many years, that their spouse knows what they are grateful for. As one couple told me, "We don't let a day go by when we don't communicate what we appreciate about each other." Be specific: "Thank you for dealing with all the awful politics at work, so you can support our family." Or, "thank you for driving the kids around after school every day. I know how hard it can be."

Bottom line? Many people have a misconception that marriage is the culmination of a relationship. Far from it. Marriage requires constant work and attention. Don't expect it to come easy. Talk, talk, talk. Be willing to see things from your spouse's point of view. If you can't work things out on your own, talk to a licensed marriage therapist before the problems grow too big. An experienced therapist has seen all the issues you're experiencing -- and then some -- and can help you and your spouse find strategies to work through them.
